OJS review forms

I’m experiencing an issue with review forms. There is a customized review form that migrated to the new version, the reviewers fill them, but when it is to request the revision, or send the review, it doesn’t attach the reviewer form results. That area in the email goes blank. Any idea? It seems specific of this journal.
